The Foundation only accepts applications through our online portal. Below is a list of documents you will need to have prepared:
Contact information including agency name, contact person name and title, mailing address, phone number, email address and website address if applicable.
A brief description of the history and mission of the agency.
A description of the project, including statement of need, three impact goals the project will achieve, timeframe for completion and how it supports the mission of the agency
Project budget that includes a detailed revenue and expense plan, committed sources of support, in-kind contributions and other pending grant applications. What specific amount is requested from the Foundation and how will the funds be used?
A detailed estimate of post-completion operating costs and a plan to sustain these additional expenses.
A current agency operating budget, including planned and actual revenue and expenses.
The most recent audit or financial review. The most recent IRS 990.
