Proposal Elements

The Foundation only accepts applications through our online portal. Below is a list of documents you will need to have prepared:

• Contact information including agency name, contact person name and title, mailing address, phone number, email address and website address if applicable.
• A brief description of the history and mission of the agency.
• A description of the project, including statement of need, three impact goals the project will achieve, timeframe for completion and how it supports the mission of the agency
• Project budget that includes a detailed revenue and expense plan, committed sources of support, in-kind contributions and other pending grant applications. What specific amount is requested from the Foundation and how will the funds be used?
• A detailed estimate of post-completion operating costs and a plan to sustain these additional expenses.
• A current agency operating budget, including planned and actual revenue and expenses.
• The most recent audit or financial review. The most recent IRS 990.
• Copy of IRS determination letter.
• Annual report if applicable.
• Board of Directors list.
