Proposal Elements
The Foundation only accepts applications through our online portal. Below is a list of documents you will need to have prepared:
- Contact information including agency name, contact person name and title, mailing address, phone number, email address and website address if applicable.
- A brief description of the history and mission of the agency.
- A description of the project, including statement of need, three impact goals the project will achieve, timeframe for completion and how it supports the mission of the agency
- Project budget that includes a detailed revenue and expense plan, committed sources of support, in-kind contributions and other pending grant applications. What specific amount is requested from the Foundation and how will the funds be used?
- A detailed estimate of post-completion operating costs and a plan to sustain these additional expenses.
- A current agency operating budget, including planned and actual revenue and expenses.
- The most recent audit or financial review. The most recent IRS 990.
- Copy of IRS determination letter.
- Annual report if applicable.
- Board of Directors list.
